What is Home Health Care?

Home Health care refers to medical care provided in a person’s home. Essentially a qualified professional such as an LPN, CNA, HHA, or nurse comes to a person’s home to provide a wide range of care such as assistance with daily activities, nursing care, physical therapy, occupational therapy, speech therapy, and other types of medical treatment and support. Depending on the level of care needed an aide may work a few hours a day or live in the home of the individual to provide 24-hour care. 

Who is Eligible for Home Health Care ?

You might be eligible to receive home health care under the Medicare if:

  • Your doctor prescribes home health care for you.

  • You need either skilled nursing care or therapy (physical/occupational/speech therapy) on an intermittent basis.

  • Your condition limits your ability to leave home, rendering you “homebound.”

Who is Eligible for Home Health Care with Medicaid ?

Under the NJ Family Care - New Jersey residents with medicaid are eligible to receive home health care services at no cost. Home and community based services (HCBS) are designed to help Medicaid recipients continue to live in their own homes or community . HCBS are commonly provided through Medicaid waivers, which are a way for states to offer programs not traditionally covered by Medicaid.

Is your agency accredited?

Yes, we are proud to be accredited by the New Jersey Commission on Accreditation for Home Care (CAHC). This accreditation reflects our commitment to providing top-quality home healthcare services throughout Northern New Jersey.

Are your staff members licensed and trained?

Absolutely! Our home health aides are Certified Home Health Aides (CHHA). Additionally, we launched our Wellcare Academy in 2020 where we are training and certifying the next generation of compassionate caregivers. We are committed to providing the highest standard of care and professionalism to our community.

How do you create a care plan?

At Wellcare Nurses, we believe that our holistic approach ensures that members recieve the best care possible. We start with our initial phone screening where we learn about the needs of our member. Our Nurse will conduct a professional skilled evaluation of your needs. We collaborate closely with you and your caregivers to develop a personalized care plan that addresses your individual healthcare needs

Do you accept insurance?

We work with a range of NJ Family Care insurance providers including : United Healthcare , Aetna, Amerigroup, Horizon NJ Health, Veterans Affairs. We also work with benefit coordinators to help prospective members understand what options are available to them. Please contact us to discuss your insurance coverage and how it can be applied to our services.

What is the cost of your services?

Rates may vary by location. On average members can expect to pay anywhere from $32 - $45 / hourly Contact a Wellcare Nurses to discuss details in your area.

What areas do you serve in Northern New Jersey?

We provide our home healthcare services throughout various counties in New Jersey : Essex County, Bergen County, Union County and Passaic County. We are always expanding our services to new areas to ensure that our services are accessible to all.
